How do I see who sent a specific survey or evaluation?

Please look for the name of the evaluation title under the Evaluation Records tab. Next, visit Administrative Tools > Evaluation Templates and search for the same title. The result would show you the name of the evaluator creator along with the evaluation type, frequency and trigger date.
