Why did an employee only receive an evaluation for their goals and not the other result areas? Is there a way to tell who sent an evaluation and what template was used?

The evaluation may have been set up to only include goals as the result areas. To include other result areas, please make sure to add them from the result area selection when setting up the evaluation templates. To see who sent out the evaluation, you can check the name of the evaluation creator on the Evaluation templates listing page.
