Please visit Administrative Tools > Result areas and click on the blue button “Add a new result area” on the upper right side of the page. Enter a name for your review form, and click the “Add a new performance indicator” button to start adding your questions. You can set up questions with a customized rating scale using “Selections with scores” under Select type; or “Selection without scores”; or “Free-text input”. Make sure you Save any changes! You can also customize any of the Result Areas within the library by clicking on the green Edit icon. You will then put it all together under the Evaluation templates to send out the evaluations.
